I believe simply called triple stitch Zig Zag.

The thread is Bainbridge V69

They don't tie the ends together they simply go back and overlap the area just before the break and then move on.

I can't help with machine settings...

Minor repairs to damaged threads are pretty easy to repair. I've done this type of repair plenty of times. Just make sure the panels are lined up, remove any loose or damaged threads, and then use a hand needle to carefully stitch back through the existing holes. Start a few stitches back from the damage (usually about 3 or 4 stiches) so the new thread overlaps the old. It helps to have a pair of pliers handy to push/pull your needle through since the fabric can be quite stiff.
