The cleat on my main sheet Swivel Cam Cleat w/ Bullseye is not moving smoothly and I've noticed that the plastic is breaking down and the bearings are shot.

Can anyone tell me what replacement cleat to get?

I think there's been Harken and Ronstan cleats used on newer boats.

I think the Harken is the 365 Standard Carbo-CamĀ® Cleat. Is that correct?

Does anyone know the Ronstan part?

Thanks!

On all the H16s I've dealt with, it's the standard Harken 150 (check the specs to make sure): https://www.harken.com/en/shop/cam-cleats-kits/standard-cam-matic-cleat-150/
The torsion springs are usually the first things to go, causing the pawls not to close well. Worn ball bearings will cause performance issues too, of course. On a very old one, the teeth on the pawls can get so worn that they do not hold.

If the teeth are the issue, you need to replace the entire cleat. Make sure it has aluminum pawls...there used to be a similar version out there with plastic ones. If the teeth are okay, you can actually rebuild it with this kit: https://www.harken.com/en/shop/cam-cleats-kits/150-cam-matic-cleat-rebuild-kit-1993-and-newer/ The rebuild instructions are included, and it's quite easy. Just make sure that you 1) put some dish liquid or such where the balls go to hold them in place until reassembly is complete (then rinse out the soap), and 2) do the procedure over a tub or something so that, when item 1) doesn't work, the balls don't spread all around your house.

The first time I blew one up, I was in a hurry and was able to buy the replacement locally. Later I got the kit, rebuilt the old one, and keep it on hand as a spare.

Hope this helps...
