Both times for me it was the leeward rear, but Sprayskirts would have stopped the front from disconnecting.

mine was windward rear right....about 90min into the trip, while floating around, not even sailing at the time. the aka came loose out of the hole, but the brace was still connected so it just folded up forward to the hull, freaked me out at first as i didnt know what just happened.

Thank you, Kayak Bob. I often carry precious cargo (wife and young children) onboard my TI. I will install a leash my rear akas immediately.

We do work hard to get them out enjoying sailing with us and don't need any negative experiences (like dumping them out of the boat) to give them a reason to say no. :)
