Happened to me constantly!
Once you get it out (which can be an issue) look at the insert. I'll bet there are very fine burrs as mine had.
Use a nail file...not a real file to clean it off or try this just because. The paper kind of file is perfect. Clean the edges and corners. Be careful not to round them off so work flat and straight.
Go to any sporting goods store and find the smallest bottle of "Break Free". A few drops before you put it in and it will slide out easy.
